Looking for a WordPress developer who will create and connect to my website an evaluation forms based on formidable forma. Looking for someone familliar with that plugin.

Forms will be evaluating cleaning status of our clients properties. Forms should be calculating % of success rate for categories (regions), and single properties. Client will be doing evaluation of cleaners works (attached sample form).
