We are looking to design a website (can be with a template from Duda or any other website building company) but it needs to be functional as an online store for esthetic products and also have a large portion dedicated to consultation where people can fill out forms and questionnaires.  A good example of a website that is similar is:

If you have done similar projects (doesn’t have to be the same industry), then that would help greatly in deciding who to use for this project.  We are looking to have it finished by the end of June.  Thank you!
