We are looking to create a platform for conducting different sets of assessments which should result in a report being generated.

These assessments are done at organisation level, team level and individual level.

Each assessment may have a set of elements. Each elements may have set of categories and each category may have set of questions (open text, multiple answer, radio, yes/no, range, etc.)

1) After completing the questionnaire, the respondent should receive a thank you and a details infographic report.
2) We should be able to show team level report as well organisation level reports.
3) We should be able to customise the questions and should have user access for organisations.

Who should apply?

We are looking for a team of people who can deliver this and then can help maintain it on a recurring basis.
