– Create a Shopify plugin ready for app store deployment that allows customers to participate in an optional survey during checkout
– Add a yes/no checkbox at checkout. If yes is selected, the checkout page serves a question immediately below the checkbox with a Submit button, accepts text entry from a customer, and saves the entry and customer to a downloadable CSV or Google Doc when the customer clicks the Submit button (I am open to suggestions if there is a better way to store this data, for example directly within Shopify)
– When the Submit is clicked a discount is applied to the shopping cart
