We need to develop a simple Landing Page with a web form that consists of 4 questions that appear one by one. Three yes/no questions and 1 zip code question.
Depending on the replies the final page will show whether the user is qualified for the program or not.
If yes, it should display the active phone number that the user can click and automatically call that number (if the user uses a smartphone/tablet).
The final page should provide a different phone number depending on the zip code.

The example of the page is below. We need to duplicate all functionality of that landing page and make sure it’s responsive for mobile devices.
