We are looking for someone to develop the following design into a fully functional web form. The link to the figma design is below. We also need each of the form answers to be posted to a google sheet and each instance of the form needs to be able to have its own unique ID: Then in the google sheet there needs to be a script that will send the form results via Email and SMS to the email addresses associated with that ID. This form will need to be able to stand alone on it’s own URL as well as be embedded on the users website. Using both a direct embed and a pop-up embed.
