We are looking for a skilled professional to modify an existing form to create a  ballot that can be distributed via Docusign. This ballot will be used for an upcoming HOA vote for Capital Improvement Projects, there are 3 items that require either a  YES or NO vote, then the Homeowner information on the bottom. The project will involve the following tasks:

– Integrating the ballot with Docusign (or providing instructions to do so)for easy distribution and collection of votes
– Testing the system for usability and functionality

Skills required:
– Proficiency in Docusign and electronic signature software

This is a one time project
