I need some custom plugins developed which will add price attributes to some of the standard controls.
The controls I require are:
– Check box with a price,
– Check box group with a price for each item
– Radio Button with a price for each item
– Quantity control with a price  
– Total control, which adds up all of the other price controls on the form.

The attached image gives you some idea of what the final output should be able to do.

This is a short term contract as the internal development team does not have availability at present, but there may be more work available to the right person.

This output should be a .js file for each control which can be added and registered with formbuilder using the builtin custom plugin methodology.

Hourly Range: $10.00-$25.00
